چکیده

INTRODUCTION Cholesterol granulomas (CGs) are rare benign cystic lesions containing cholesterol crystals in a gluelike fluid and are frequently found in the petrous apex (PA) and in the nearby anatomical area. The gold standard treatment of PA CG counts on its surgical drainage and ventilation to prevent recurrence. Different surgical approaches have been described in the past to achieve drainage of these lesions, either through lateral, or more recently through an endoscopic endonasal corridor. In this report, we describe an innovative technique used to keep open the site of drainage, using a vascular pedicled nasoseptal flap (Hadad-Bassagasteguy flap) after an endoscopic trans-ethmoid-pterygoid-sphenoidal approach to a petrous apex PACG.

Endoscopic transsphenoidal surgery for cholesterol granulomas involving the petrous apex.

Surgery for cholesterol granulomas involving the petrous apex has traditionally been performed via a lateral skull base approach. We present a case-series of four cholesterol granulomas treated through the endoscopic-transsphenoid approach over the last 10 years. Drainage was successful and symptomatic improvement was obtained in all cases (follow-up 6 months-10 years). Primary 'sphenoid' lesio...

Endoscopic Endonasal Management of Recurrent Petrous Apex Cholesterol Granuloma

Introduction Petrous apex cholesterol granulomas (PACGs) are uncommon lesions. Recurrence following transcranial or endonasal approaches to aerate the cyst occurs in up to 60% of cases. We describe the technical nuances pertinent to the endonasal endoscopic management of a recurrent symptomatic PACG and review the literature. Results A 19-year-old woman presented with a recurrent abducens nerve...

Upper Nasopharyngeal Corridor for Transnasal Endoscopic Drainage of Petroclival Cholesterol Granulomas: Alternative Access in Conchal Sphenoid Patients

Background Cholesterol granulomas arising at the petrous apex can be treated via traditional open surgical, endoscopic, and endoscopic-assisted approaches. Endoscopic approaches require access to the sphenoid sinus, which is technically challenging in patients with conchal sphenoidal anatomy. Clinical Presentation A 55-year-old woman presented with intermittent headaches and tinnitus. Formal au...

A case of petrous apex cholesterol granuloma successfully treated with endoscopic endonasal surgery.

A transcranial approach in combination with a transpetrosal setting has been the mainstream of surgical treatment of cholesterol granulomas in the petrous apex. However, endoscopic endonasal surgery has become a choice of treatment for these lesions with recent advancements in surgical techniques and instruments. We report a successful case of cholesterol granuloma managed with endoscopic endon...

In reference to endoscopic-assisted transmastoid decompression of petrous apex cholesterol granuloma.

It was with great interest that we read the article by Carlton et al. describing a surgical pearl for gaining access to the petrous apex using a microscope and an endoscope. Using an endoscope during the lateral microscopic approach to the petrous apex is still the subject of debate. Here, we would like to emphasize the advantage of endoscopes not only in cholesterol granulomas but also in othe...
